ITV Ventures™ Independent Business Owner (IBO) Terms & Conditions

I want to become an Independent Business Owner (IBO) in ITV Ventures™. I am of legal age in the state I live in and authorize ITV Ventures™ to charge my credit card in the amount of $165 (One Hundred Sixty Five Dollars) for the ITV Ventures™ Basic IBO Package. I understand that this is the only requirement to become an Independent Business Owner (IBO) with ITV Ventures™.

I authorize ITV Ventures™ to charge my credit card in the amount of $334 (Three Hundred Thirty Four Dollars) for the Optional Premier IBO Package. I understand that this product package purchase is optional and contains various ITV Ventures™ products as chosen by management and may change from time to time. I understand that this purchase is not required to become an Independent Business Owner (IBO), but I am choosing to do so freely of my own accord.

I hereby authorize ITV Ventures™ to enroll me in its optional Order Assurance Program. Please send me an assortment of products totaling $100 (One Hundred Dollars) for each month that I fail to purchase or sell $100 worth of product. I authorize ITV Ventures™ to withdraw funds for this authorized program directly from my checking account via electronic check or major credit card account on a monthly basis. I understand that the Order Assurance Program (O.A.P.) that I am choosing to enroll in today is optional, but in order to be placed in the 4 x 7 Power Matrixsm it is required.

I authorize the Business Education Support Team (BEST) to charge my credit card or to debit my checking account via electronic check in the amount of $29.95 per month for ITV Ventures™ Monthly Action Program (M.A.P.) ongoing education and training system. I understand that this is optional, but in order to be placed in the 4 x 7 Power Matrixsm, it is required. I also understand that if I fail to participate in the MAP for two (2) consecutivemonths that this Agreement will automatically terminate without notice.

Please deduct a 2.95 fee, from my monthly bonus check for each bonus check that is sent to me.

I have read the terms and conditions of this Agreement and agree to abide by the company Policies and Procedures as stated on this agreement and in the ITV Ventures™ Policies and Procedures which are incorporated into this Agreement and understand that they may change from time to time and it is my responsibility to review and follow them.

I have read the ITV Ventures™ Compensation Plan and understand all of my options. I authorize ITV Ventures™ to charge my credit card or issue an electronic check from my account for the annual renewal fee of $99 (Ninety Nine Dollars) during the month of my anniversary of becoming a Representative.

If I do not have my credit card, then I authorize ITV Ventures™ to enroll me in the electronic check program and I hereby authorize ITV Ventures™ or its authorized agent in accordance with this Agreement to initiate debit/credit entries to my checking account as indicated. This authority is to remain in full force and effect until ITV Ventures™ has received written notification from me of its termination in such a manner as to afford ITV Ventures™ reasonable opportunity to act on it pursuant to this Agreement. As a convenience to me, I hereby request and authorize ITV Ventures to withdraw funds from my checking account using the electronic check program. Checks made on my account and payable to the order of ITV Ventures™ are valid provided there are sufficient collected funds in my account to pay such checks upon presentation. This authority is to remain in effect until ITV Ventures™ actually receive such notice; I agree that ITV Ventures™ shall be fully protected in honoring such check. I further agree that if any such check be dishonored, whether intentionally or inadvertently, ITV Ventures™ shall be under no liability whatsoever even though such dishonor results in the forfeiture of product, bonuses or privileges I may have been entitled to as an Independent Business Owner (IBO).

ITV VENTURES™ INDEPENDENT BUSINESS OWNER (IBO) AGREEMENT TERMS OF ENROLLMENT

As an Independent Business Owner (IBO) with ITV Ventures™, I acknowledge that I am an independent contractor and not an agent or employee of ITV Ventures™.

As an Independent Business Owner (IBO) with ITV Ventures™, I will make no statements, disclosures, or representations to sell ITV Ventures™ products or services, or in recruiting other prospective Independent Business Owners (IBOs), other than those contained in approved ITV Ventures™ literature.

I agree that as an ITV Ventures™ IBO, I shall place primary emphasis upon the obtaining of retail customers who are not IBOs, and will resell over 50% of my wholesale product purchases to non-IBO consumers prior to ordering additional wholesale products.

I understand that no purchase other than an ITV Ventures™ Basic Package is necessary to become an Independent Business Owner (IBO) with ITV Ventures™.

I understand that if I do not purchase or sell at least $100.00 worth of products for two (2) consecutive months, this Agreement shall automatically terminate and my Independent Business Owner (IBO) distributorship will be cancelled and any Bonuses will be forfeited. I also understand and agree that if I do not purchase or sell $100 worth of products for one month that I will be required to purchase or sell $200 worth of products the following month.

I understand that in order to maintain a viable marketing system and to comply with changes in applicable laws ITV Ventures™ reserves the right to change prices, company policies, company literature, and/or the compensation plan, without prior notice.

I understand that this Agreement is governed by the laws of Massachusetts. I aknowledge that all parties agree that any claim, dispute or other difference between them shall be exclusively resolved by binding arbitration pursuant to the Commercial Arbitration Rules of the American Arbitration Association with arbitration to occur at Beverly, Massachusetts. Louisiana residents may choose Louisiana law and arbitrate in New Orleans.

I understand that this Agreement shall be deemed in effect upon its receipt and acceptance by ITV Ventures™.

ITV VENTURES™ ORDER ASSURANCE PROGRAM (OAP) TERMS AND CONDITIONS

I may cancel the optional Order Assurance Program at any time by sending a written notice to ITV Ventures™bearing my signature, printed name, address, and IBO number. This cancellation will be effective the month after it is received by ITV Ventures™.

I have the right to receive written notice (mailed or delivered at least 10 days prior to the scheduled transfer date) of the amount and scheduled date of all transfers varying from the previous transfer relating to this authorization of the pre-authorized amount.

ITV Ventures™ cannot obtain more funds from my account (on a monthly basis) than those I have pre-authorized.

I understand that ITV Ventures™ will charge $5.95 (plus Shipping & Handling) per month for all OAP transactions. This processing fee is included in the amount authorized on the OAP section of this agreement.

Credit Card and Electronic Check Program: If I fail to purchase or sell $100 worth of products during the calendar month, the OAP Program will go into effect. This means my credit card will be charged or money deducted by the electronic check program, in the amount of $100 plus a $5.95 processing fee, plus appropriate shipping and handling. I will be sent an assortment of products as chosen by management equal to or greater than $100.

If my credit card or electronic check does not clear on the day withdrawals are made, I will not have a credit month order. ITV Ventures™ is not responsible for notification.

ADDITIONAL TERMS & CONDITIONS

I understand and agree that all tapes, seminars, training materials and brochures offered by the Business Education Support Team (BEST) and ITV Ventures™ are optional and not required unless otherwise stated in the Policies and Procedures. I may or may not purchase these items based on my own choosing, and the purchase of these items is nonrefundable. ITV Ventures™ will honor all state buy back laws in Georgia, Louisiana, Maryland, Massachusetts, Montana, Puerto Rico, Oklahoma, Texas and Wyoming. Additionally, Montana IBO's who cancel within 15 days are entitled to a 100% refund of any consideration given to participate.

I aknowledge that reports referring to large earnings of successful Independent Business Owners (IBOs) are not necessarily typical or representative of what individual Independent Business Owners (IBOs) may earn. INDIVIDUAL EARNINGS ARE STRICTLY DEPENDENT UPON INDIVIDUAL TIME, EFFORT AND ENTERPRISE IN THE PARTICULAR AREA IN WHICH THE INDIVIDUAL INDEPENDENT BUSINESS OWNER (IBO) WORKS OR LIVES.

I agree and UNDERSTAND THAT IN ORDER TO BE SUCCESSFUL IN THIS VENTURE I MUST PURCHASE AND SELL ITV VENTURES™ PRODUCTS AT RETAIL AND SPONSOR OTHER INDEPENDENT BUSINESS OWNERS (IBOs) TO DO THE SAME; AND IN ORDER FOR THEM TO BE SUCCESSFUL THEY MUST PURCHASE AND SELL PRODUCTS AT RETAIL, AND RECRUIT OTHER PEOPLE TO DO THE SAME, AND SO ON.

I agree to receive notifications and solicitations by ITV Ventures™ or its affiliated companies by email, text message, automated telephone messages or by any other means existing now or created in the future
